Appointments

I am in solo practice with no secretarial or nursing back-up. Thus you will need to be an active partner in your care, to help minimize paper work and call-backs. I will not file insurance except medicare. I will help fill out forms e.g. diagnosis, documentation of visits etc, but you will have to interact with the carriers yourself. Payment is expected at the time of service, check or cash [ please have the correct amount]

New patient $160

Medication management $40

Therapy negotiated per unit time

My confidential land line is 865 579-4534, it will be answered once a day  Monday – Friday. This is the number to call for more information or to make an appointment. For same day messages ,ONLY on the days of your appointment, call 865 898-3105. This number is to be used for same day cancellations, running late etc. not routine messages.If necessary to cancel or reschedule , please do so at least 24 hrs in advance. 
On your first appointment you will be asked to fill out several forms , so please plan to arrive 15-20 minutes early.
